[Note to school districts: With this questionnaire, we are attempting to get comprehensive data on pesticide use policies and practices within the district. We realize that (especially with larger school districts) pest control responsibilities and pesticide application records may reside at several different locations within the district (e.g., grounds maintenance department, facilities maintenance department, environmental health and safety office), or with a pest control contractor. We are hoping that you can help us identify and get a copy of this questionnaire to all of the persons who may have partial records or information about pesticide use in your district.
